I am happy to discuss any questions you have regarding the information and policies provided. 1. Qualifications: I am a Registered Nurse licensed in the state of Texas since 1996. I comply with the Texas Nursing Practice Act, the Patient Bill of Rights and continuing education requirements. Through advanced education I have received certification in clinical nutrition through the Clinical Nutrition Certification Board (www.cncb.org). Maintaining certification requires attending 2 of 3 IAACN Annual Scientific Symposiums, 40 continuing education hours every 2 years, and re-certification every 5 years. 2. Fees: My fees vary according to the length and type of consult you choose. Please see the tab "your visit" for details.
If cancellation is necessary, please give 48 hours notice by phone or e-mail (no text messages please) as provided above and there will be no charge. 3. Nutritional Therapies: The theory behind nutritional therapies involves acknowledging that the foods you eat, the stress you’re under, your lifestyle choices, your environment and your genetics all affect your reproductive health. By teaching how the body functions and the proper food and lifestyle choices needed to regain and maintain health, nutritional therapies aim to support your body’s inherent ability to heal. Therapies or supplements I recommend are not for treating disease, but rather to support your body’s goal of wellness. 4. Understanding your assessment and recommendations: Nutrition works on the body at the cellular level. Changes in your nutrient intake and lifestyle may take 3 months to manifest, but yield a lifetime of benefits. Nutrition can be incorporated into any current fertility treatment to improve your body’s response. The success of any therapy is directly related to your level of participation. That stated, it is solely your choice to use or purchase any therapy or product I recommend. I can be reached through e-mail at any time to clarify my recommendations. I cannot guarantee you a pregnancy or a healthy child. What I do offer is an improved chance for a pregnancy and a lifelong interest in the nutritional well-being of yourself and your family. 5. Confidentiality: Please read the enclosed Notice of Privacy Practices – Short Form. If you have any questions or would like a full legal copy of HIPAA please contact me. All transactions and records are kept strictly confidential. Your records may be released only when requested in writing by you, or when required by law. Under the HIPAA privacy rule patients are given the right to request confidential communications. Please notify me of updates to the following information. 6. Primary Physician Care: You will continue to consult your primary care physician or seek the advice of a specialist if your primary care physician or Rosalind Haney advises you to do so.
